Alaca, a young and innocent girl born out of an extramarital relationship in a modest family, lives without knowing her true origin. Her life takes a dramatic turn when the daughter of her wealthy biological father needs a kidney transplant. The day her kidney is forcibly removed becomes the catalyst for her quest for truth, and she discovers that even the love of her life may be hiding the truth from her.

Kenan, who grew up as an orphan alongside his brother Gökhan, owes his medical career to Baki, who supported them throughout the years. Unaware of the connection between Alaca and Baki, Kenan finds himself caught in performing an underground surgery to repay his debt to Baki. When it is revealed that the patient is Alaca, Kenan must choose between challenging the ruthless Baki or helping Alaca find answers. Things get even more complicated when Gökhan is involved in a hit-and-run incident, of which Baki has evidence.
